| To be
awarded inservice points, participants must complete and submit both
the Staff Development Evaluation form
and the Inservice Follow-up
form. The Staff Development Evaluation
form may be completed and submitted on-line. The Inservice
Follow-up form requires signatures so you must print it to complete
the information. You may then submit it to the facilitator via the
courier (no later than four weeks
after the inservice activity.) |
